Cost & Insurance ReimbursementA doctor or dentist can refer you for Physical Therapy if you have a physical problem such as lower back pain, TMJ, tendonitis, ext. The referral could read: “Lumbar Degenerative Joint Disease (DJD). Physical Therapy for ten to twelve sessions. Evaluate and treat.” Bring the referral to the first session. A typed receipt will be given to you every couple of sessions which you can submit attached to your carrier’s claim form. Most claims are processed smoothly. Ditchfield Holistic Physical Therapy does not accept assignment and is not part of any HMO, Medicare, No Fault, or Workmen’s’ compensation plans. If your insurance covers Out of Network Physical Therapy, then you should not have a problem. Payment is made at each session by cash or check. Cost is $150.00 per 90minute treatment session. Session are by appointment only. |
